What Is SS-31 (Elamipretide)?

SS-31, also known as elamipretide, is a synthetic tetrapeptide that selectively targets the inner mitochondrial membrane by binding to cardiolipin, a phospholipid essential for mitochondrial structure and energy production. By stabilizing cardiolipin, SS-31 may reduce oxidative stress, improve ATP synthesis, and support cellular function in tissues with high energy demands including the heart, brain, and skeletal muscle. It was originally developed by researchers studying mitochondrial dysfunction in degenerative diseases and has since become one of the most studied mitochondria-targeted compounds in medicine.

In September 2025, the FDA granted accelerated approval to elamipretide (under the brand name FORZINITY) for Barth syndrome, making it the first FDA-approved mitochondria-targeted therapy. That approval validated the core mechanism: stabilizing cardiolipin improves mitochondrial function in measurable, clinically significant ways. Once there, it binds directly to cardiolipin, stabilizing the membrane structure and allowing the electron transport chain to function more efficiently. The result is improved ATP output and reduced oxidative damage at the cellular level.

Research published in GeroScience found that SS-31 specifically improves ADP sensitivity in aged mitochondria by increasing uptake through the adenine nucleotide translocator (ANT). In practical terms, aging mitochondria become better at converting fuel into usable energy, which is precisely the mechanism that declines with age.

What to Expect During Core Spark Treatment

Here is how the process works from start to finish.

Step 1: Consultation. You meet with a provider at our Lincoln Park clinic or via telehealth if you are elsewhere in Illinois. They review your medical history, current medications, and goals. If SS-31 is a good fit, they write the prescription. If it is not the right protocol for you, they will tell you directly and suggest alternatives.

Step 2: Pharmacy fulfillment. Your Core Spark protocol is prepared by our 503A compounding pharmacy partner and shipped to the clinic or directly to your door.

Step 3: Treatment. Core Spark is administered as a daily subcutaneous injection. Your provider walks you through proper injection technique at your first visit. Most patients self-administer at home after that initial instruction. The protocol is available in two options:

4-Week Protocol

A single month of daily SS-31. Good for patients who want to evaluate how they respond before committing to a longer cycle.

8-Week Protocol (Recommended)

The standard cycle that allows the full range of benefits to develop. Research and clinical experience suggest that mitochondrial improvements compound over time, with the most meaningful changes occurring in weeks 4 through 8.
